人羧酸酯酶2（hCE-2）可识别具有大醇基和小酰基的底物。由于在活性位点口袋中存在构象干扰，其底物特异性可能受到酰基-hCE-2缀合物形成能力的限制。羧基酯酶可催化几种含酯的药物和前药的生物转化，例如血管紧张素转化酶抑制剂（替莫卡普、西拉普利）、抗肿瘤药（卡培他滨）和麻醉药。
